On-Line Cave Report Form

The work of the Indiana Cave Survey begins with a caver filling out a Cave Report Form. The data entered into this two-page form becomes an entry in the growing ICS database. Many popular caves have never been officially reported. Your casual recreational trip might fill a hole in the database if you will take the time to fill out a form and send it in. Never assume that the cave you visit has been thoroughly documented. Even if it's been reported in the past, over a number of years, conditions change and your recent observations may add a unique perspective to our knowledge.

You can download a Cave Report Form specifically formatted for your printer. After you've saved the file to your computer's hard drive or a disk, simply copy the file to a compatible printer with the command COPY CAVEFORM.XX LPT1, where "XX" indicates the specific printer file. You may have to add a more specific file location or change your command to LPT2, depending on the hardware configuration of your computer.

After completing the form, send it to the address at the top of the front page. It will be routed to the appropriate County Director who will then review the information and add it to the database. Your form is then returned to the ICS Archivist who will add it to the paper files.

A PDF file is viewed and printed by using Adobe Acrobat Reader. If you already have Acrobat Reader on your computer, selecting a PDF link will launch the program and open the file. If you do not have Acrobat Reader, you will be prompted to SAVE the file. You can download the latest version of Acrobat Reader from the Adobe website at www.adobe.com.
