It is requested that National Speleological Society (NSS) approval be granted to the following organization, to be

This application must be signed by the initial members of the applicant organization.  At least five are needed for a Grotto and ten for a Section. If some individuals have  just filed applications for NSS membership, write "Pending" in space for NSS Number. 
It is understood and agreed that, if approval is granted, this organization will comply with all provisions of the NSS
Constitution and Bylaws, and will furnish a report of its activities  to the NSS IO Committee once each year.
